WSCC Intensive Planning Team

About

The Intensive Planning Team works with identified children and young people with SEND who are at high risk of their education placement breaking down and/or at risk of needing an independent, residential school, away from their families and community.

What we do

  • We are a small group of professionals with expertise across Education, Health and Social Care
  • We support the network around the child/young person and their family to identify creative ways to achieve good outcomes and develop positive and person-centred solutions, including the identification of alternative support and funding streams.
  • We ask that the Graduated Approach has been applied over time and evidenced as unsuccessful.
  • Referrals are through professionals only
  • We require family/carer and school to give their consent for information to be shared with project partners, are actively engaged with us and are open to change and doing things differently
